Outreach Worker Position

The Town of Raynham is seeking one outreach worker to serve the local senior community.

The position will be 18 hours per week on week days to be determined.

The outreach worker responsibilities include establishing and maintaining contact with Raynham’s age 55 and older population, providing direct assistance when appropriate, or information and referrals when required.

The outreach worker must be able to make information available to clients, with cogent explanations if necessary, and make appropriate referrals on a timely basis to other agencies, including legal assistance, veterans services, Meals on Wheels, food pantry, Food Stamps, Mass. Commission for the Blind, senior housing, and others.

The outreach worker must be a certified SHINE counselor. The position also requires knowledge of and ability to fill out paperwork for fuel assistance programs administered by Self Help, Inc.; ability to work closely with area agencies on aging including, but not limited to Bristol Elder Services, and other agencies, including Salvation Army, St. Vincent De Paul, and state agencies dedicated to elder health and safety; ability to work closely with local public safety and health personnel.

The outreach worker must keep accurate written records on all clients, and ensure they are properly locked and kept private. General monthly reports on the number of clients and actions undertaken are submitted to the Council on Aging at its monthly meetings.

The outreach worker by necessity must recognize the need for confidentiality in dealing with all clients.

Qualifications for the position of outreach worker are a bachelor’s degree in human services or five years relevant experience in social work or human services, including seniors; excellent computer skills and ability to learn new software; ability to work with minimum supervision.

The outreach worker consults with and reports to the Director of the Council on Aging.
